Licensure with the State of Michigan

You MUST be licensed with the state of Michigan to practice occupational therapy or to use the protected titles of the profession including OT, OTR, OTA, and COTA.


There is a wealth of information on the State of Michigan OT Licensure  webpage:

From this page you can download an application packet, review the OT section of the Public Health Code, find out who is on the OT Board for the State of Michigan, when their meetings are and agendas/minutes of previous meetings.

To obtain an application for licensure or to contact any of the professional Boards, email the Bureau of Health Services at bhphelp@michigan.gov or call (517) 335-0918.

To verify if an individual is licensed in Michigan or if there has been disciplinary action taken on a license, go to
www.michigan.gov/verifylicense  or call (517) 241-9427.

The
Public Health Code and Administrative Rules for OT are available to view or download. If you prefer hard copies, call (517) 335-0918 (prompts 1-0) or email the bureau of health Services at bhserinfo@michigan.gov

To verify licensure to another STATE, request must be submitted in writing. Include the name/address of the other state board, your name, license number, signature and a $15.00 check or money order payable to the STATE OF MICHIGAN. Mail request to CIS-BHSER, PO Box 30670, Lansing, MI 48909. Normal processing time is 20-30 business days.

When a healthcare professional applies for their initial Michigan license, the license  is valid only until the next renewal period for that profession.

Renewal applications for each registration are sent out 60 days prior to the license expiration date.
